Pursuant to the Office Memorandum No. 306, series of 2020, from the SUC President III, the submission of credentials of the incoming first year students and transferees shall be in any of the following manners:
- By registered mail, LBC, JRS, among others or;
- To hand carry the credentials placed in a sealed envelope to be received by the guard on duty.
The credentials to be submitted are the following:
For First Year Students:
- Original copy of Report of Grades
- Original copy of Good Moral Certificate
- Clear photocopy of PSA/NSO Birth Certificate
- 1 piece 2×2 recent picture
For Transferees:
- Original copy of Honorable Dismissal/Certificate of Transfer
- Original copy of Good Moral Certificate
- Original copy of Transcript of Records (For evaluation/informative)
- Clear photocopy of PSA/NSO Birth Certificate
- 1 piece 2×2 recent picture
The deadline of submission will be on or before October 9, 2020.
